The annual Worksite Health Screening event has been scheduled for USCB employees, retirees, and covered spouses. All USCB employees and retirees currently subscribed to the State Health Plan (Standard/Savings) and their covered spouses are eligible to attend. The available services will include Biometric Screening and Optional Screening Testing List. Optional Tests are offered for a fee. You will need to pay for those with a CHECK. They cannot accept cash or debit/credit cards.
In addition to measuring your height, weight and blood pressure, the Biometric screening will include a lipid panel, chemistry profile, and hemogram. There will be no cost to you. Test results will be mailed to your home within 14 business days.
Those unable to attend one of the scheduled Worksite Health Screening events may take the Preventive Health Screening Voucher along with your State Health Plan insurance card to one of the participating screening providers and ask for the preventive health screening.
A 12-hour fast is required prior to the screening but you may have water to take any required medications.
Forms and instructions for completion will be emailed to you when your appointment is confirmed. Please complete and print these forms and bring with you to your scheduled appointment.
